A member over at Gunnit posted these pictures of a S&W M&P AR-15 that had a catastrophic failure at the range. The owner of the rifle apparently used his own reloads in the rifle. Luckily no one was seriously injured.
I imagine if it wasn’t in full battery, the lugs would have failed, and it would have been less likely to blow, because gas would have dumped out of the cylinder earlier.

That was either a barrel obstruction or a double charge.
The lugs on the barrel extension appear to be intact, as does the bolt, indicating that the failure was caused by too much gas in the carrier.
